Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.

When you come for your appointments, please remember to bring the following

• Driver's license or State ID
• Insurance information
• Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits
• Medications list (if any)
• If you have had surgery elsewhere, please bring a copy of your operation report

Very often during the course of your evaluation or treatment for an orthopedic condition we will require x-rays to determine the cause of a problem or to evaluate your progress.

Insurance Information

Dr. Pearle participates in several insurance plans and is currently taking new patients. You may not have a choice in your health benefits, but you do have a choice in quality and the educated care that Dr. Pearle and his support staff provide. Even if Dr. Pearle is not an "in-network" provider, most patients are still able to see him in an "out of network" capacity. We are able to work with you and firmly believe financial roadblocks should not interfere with your choice of quality patient care. When you call for an appointment, please have your insurance card readily accessible, so we may help you understand your benefits as they apply to seeing Dr. Pearle. Our staff is highly trained to assist you with this and may answer any questions that you may have.

In addition to insurance plans, Dr. Pearle also participates with Medicare, Workman's Compensation, and No Fault. Please note that Dr. Pearle only sees Workman's Compensation and No Fault patients with a referral from a qualified Physician, and a recorded date of accident of less than 3 weeks.

To cancel an appointment:

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
